	Michigan Department of Health and Human Services IRB Review Application All studies that propose to use the dried blood spots must be approved by the Michigan Department of Health and Human Service's Institutional Review Board. Send the completed request for IRB review to biotrust@michigan.gov
	Data Use Agreement Researchers who request data along with samples should contact The Michigan Department of Health and Human Services at biotrust@michigan.gov for information about completing this form.
	Material Transfer Agreement Before samples can be sent the requestor must sign a Material Transfer Agreement. Send the completed form to biotrust@michigan.gov
